Flag Sponsorship Options

There are four options for sponsorship: Gold, Silver, Bronze, and an Individual Flag.  

Using the link below you can view and download the sponsor form if you would like to pay by check or cash.  

If you would like to purchase online with a credit card you can click on the appropriate option in the store below.  We are capturing your address so we can send a letter acknowledging your donation.    

For both options we will need the name of your hero or group of heroes, a brief description you would like on the label, and the way you would like the sponsor name to show on the label.  You can view samples of last year sponsor entries by clicking here.
